China has 300 million smokers, nearly a third of the world’s total. The imbalance between the sexes is stark: 46.4 percent of Chinese men smoke, against 1.9 percent of Chinese women. But of course, the underlying question is, “What’s wrong with Chinese men?”

Men in China make up about an eighth of the world’s population, yet consume roughly 40 percent of the world’s cigarettes. The habit has been declining for thirty years, but slowly.

The reasons it persists have as much to do with history, economics and etiquette as with nicotine itself.

From Imperial Bans to a State Monopoly

Tobacco is not native to China. It arrived in the 1570s, brought from Luzon in the Philippines by a Fujianese merchant named Chen Zhenlong, and spread as a pipe habit believed to ward off cold and damp.
烟草并非中国原产。它于16世纪70年代由福建商人陈振龙从菲律宾吕宋岛引入,最初以烟斗吸食的形式传播开来,人们相信它能驱寒祛湿。
The response from the throne was immediate and severe. The Ming dynasty’s Chongzhen Emperor banned smoking in 1637, reportedly after a folk rhyme convinced him it foretold rebellion, only to rescind the order when it emerged his own troops fighting the Manchu were too addicted to fight without it.

The Qing dynasty tried again, extending the penalty for smoking to execution. Neither ban held.

By the early twentieth century, the habit that had survived two dynasties’ death penalties found its modern form through the British American Tobacco Company. By the 1910s and 20s, BAT controlled some 80 percent of the Chinese cigarette market.

Mao Zedong was a lifelong heavy smoker, reportedly favouring the British-made State Express 555 brand through the civil war years. By his physician’s account, he was smoking it on the day he proclaimed the People’s Republic in 1949.

That state involvement never really ended. China National Tobacco Corporation, founded in 1982, now holds roughly 98 percent of the domestic cigarette market and is the largest tobacco company in the world.

Its combined tax and profit contribution to the central government reached ¥1.58 trillion in 2025, around 7 percent of total national revenue, comparable to China’s entire annual defence budget.

Knowing, but Not Quite Believing It
知道,却未必真的相信
Neither explanation on its own holds up well against the data. Chinese smokers are not simply unaware that smoking is dangerous, and social pressure alone does not fully explain why so few who want to stop actually do.
单独来看,这两种解释都站不太住脚。中国烟民并非完全不知道吸烟有害,而单纯的社会压力,也不足以完全解释为什么这么多想戒烟的人最终没能戒掉。
What the surveys show instead is a narrower, more specific gap. Around 80 to 95 percent of Chinese adults know that smoking causes lung cancer in general terms, a figure that has held steady for years.
各类调查显示的,其实是一个更狭窄、更具体的认知缺口。约80%至95%的中国成年人知道吸烟总体上会致肺癌,这一比例多年来相对稳定。
Awareness of the specific mechanisms is far lower. Depending on the year and survey, only 27 to 46 percent of Chinese adults know smoking causes stroke, and only 38 to 43 percent know it causes heart attack, both well below the same figures in countries where smoking has fallen furthest.

Only 14 percent, in one national survey, knew that low-tar cigarettes are no safer than ordinary ones, a specific and consequential piece of ignorance that keeps many smokers switching brands rather than quitting.

Where knowledge is genuinely present, researchers studying Chinese male smokers specifically have identified a separate obstacle: a set of self-justifying beliefs, known in the research as smoking rationalisation, that let a smoker acknowledge the risk in the abstract while still not applying it to himself.

In one national study, 69.8 percent of male smokers agreed that smoking relieves fatigue, 69.7 percent that it relieves stress, and 63.4 percent that it eases socialising; 59.8 percent pointed out that plenty of non-smokers also get lung cancer.

One belief identified in this research appears specific to China: a traditional-medicine notion that a body long accustomed to smoking has reached its own internal equilibrium, and that quitting abruptly will disturb that balance and cause harm rather than prevent it.

Separate research using standard models of health behaviour has found that a smoker’s confidence in his own ability to quit successfully, and his belief that quitting is genuinely worth the disruption, predict his intention to quit more strongly than fear of disease itself does.

Put plainly, most Chinese male smokers who continue do so neither purely out of ignorance nor purely under outside pressure, but because a genuine, if incomplete, awareness of the risk coexists with a set of private beliefs, that it relieves real stress, that quitting itself carries a cost, that the risk applies more to others than to him, which the outside pressure of a business dinner or a family gathering never has to overcome on its own.

The Balcony Compromise
阳台上的妥协
The scenario is well documented, not anecdotal. Chinese wives object to their husbands’ smoking often, and the home has consistently been found to be the single largest source of secondhand smoke exposure for both women and children, ahead of workplaces, government buildings and schools.
这一场景有据可查,并非个别现象。中国妻子反对丈夫吸烟的情况相当普遍,而家庭也一直被证实是女性和儿童接触二手烟的最大来源,超过了工作场所、政府机关和学校。
Nationally, 68.1 percent of non-smokers in China report secondhand smoke exposure, and among non-smoking women of reproductive age, 65.1 percent report exposure at home specifically from a husband who smokes.
在全国范围内,68.1%的非吸烟者报告曾接触二手烟;在育龄非吸烟女性中,65.1%报告曾在家中接触到丈夫吸烟带来的二手烟。
Wives do act on this. Interview-based research in Shanghai found that roughly half of wives had directly tried to persuade their husbands to quit or cut down, and among pregnant women specifically, 69.4 percent said they had asked their husbands outright not to smoke in their presence.

What that pressure typically produces, though, is a compromise over location rather than a change in behaviour. In one qualitative study of smokers’ households, none had adopted a complete home smoking ban; roughly half had only partial restrictions, smoking confined to a balcony, a doorway, or outside altogether.

Researchers identified the same barriers each time such bans were proposed and failed; the general social acceptability of smoking, the awkwardness of asking guests who smoke to stop, and what the study’s authors described plainly as the authoritative attitude of the husband or father-in-law within the household.

The clearest evidence of that imbalance comes from an intervention study that tracked how successfully wives could get various smokers to stop smoking around them. Their confidence and success at stopping other smokers in the household, in-laws, other relatives, visiting guests, rose significantly. Their success at stopping their own husbands did not move at all.

Part of the explanation is the wives’ own restraint. Researchers studying these households found that many women, while genuinely worried about the health risk, were reluctant to press the point too hard for fear of disrupting family harmony, so persuasion tended to stay as a repeated, gentle request rather than an ultimatum.
其中部分原因在于妻子自身的克制。研究这些家庭的学者发现,许多女性虽然真心担忧健康风险,却因害怕破坏家庭和睦而不愿把话说得太重,因此劝说往往停留在反复、温和的请求,而非最后通牒。
The outcome is the arrangement many foreign observers of China notice: a husband smoking alone outside, phone in hand, for as long as it takes. It resolves the immediate domestic friction without requiring him to give up the habit, and it leaves untouched the private beliefs, that it relieves his stress, that quitting would upset some internal balance, that covered earlier, which the balcony was never going to challenge in the first place.
由此形成的,正是许多在华外国观察者常常注意到的场景:丈夫独自在户外,手机不离手,一抽就是许久。这种安排化解了眼前的家庭摩擦,却不要求他真正戒掉这个习惯,也丝毫没有触及前文提到的那些私人信念——吸烟能缓解压力、戒烟会打破身体的某种平衡——而这些信念,本来就不是阳台能够解决的问题。
The Ban Nobody Enforces
没有人真正执行的禁令
The premise is worth testing before explaining it. China enforces plenty of rules with real rigour; traffic checkpoints, real-name registration, security screening, but a public smoking ban turns out to run on a completely different enforcement mechanism, and that difference explains the mall toilet.
在解释这一现象之前,先要检验一下这个前提本身。中国确实有许多规则执行得极其严格——交通检查站、实名登记、安检——但公共场所禁烟令依赖的却是一套完全不同的执行机制,而正是这种差异,解释了商场厕所里发生的事。
China’s first nationwide indoor smoking ban dates to 2011. It carried no attached penalties, and by the government’s own later admission, it was poorly enforced almost everywhere outside a handful of cities.

As of 2022, only 24 Chinese cities had adopted comprehensive smoking bans. The rest still permit designated smoking rooms or areas whose size, ventilation and airtightness requirements are defined so loosely that smokers and inspectors alike often cannot say for certain what counts as compliant.

Even where bans exist, the penalties have historically been trivial. Beijing’s fine for smoking in a banned area was, for years, just ¥10, about $1.50, and state media described it plainly as seldom enforced; a 2015 update raised it to ¥200, still under $30.

On the day Beijing’s tougher ban actually took effect, inspectors visited 565 venues and logged violations at only 147, and 146 of those received nothing more than a warning. Restaurants, the venue type closest in character to a mall food court, were the worst offenders, with 82 percent found non-compliant.

Beijing’s 2015 law was not the first attempt at exactly this, and the shape its first year took was itself an old pattern repeating. Restaurants had briefly been targeted around the 2008 Olympics, a crackdown that lapsed almost as soon as the foreign press had gone home; Shanghai made a similarly short-lived push ahead of its 2010 World Expo; and Guangzhou had already tried and abandoned restaurant smoking bans on and off since 1995.

The 2015 law’s opening weeks looked genuinely different: the WHO’s China representative described a June in which Beijing’s indoor air had visibly cleared, and health inspectors issued dozens of fines and fielded thousands of hotline complaints. But even sympathetic observers at the time flagged that the real test would come once winter arrived and smokers could no longer simply step outside without a coat.

One senior official captured the arc precisely in his own account to a state broadcaster: he did not smoke at all on the ban’s first day, was up to four or five cigarettes by the third, and afterward, in his words, everything basically went back to normal. He put the relapse down to the importance of what he called the human element in Chinese life, a reference to smoking’s role in masculine bonding and business ritual, where the offer of a cigarette can confer status in a way a warning poster cannot compete with.

The personal, informal enforcement you describe, asking a smoker directly to step outside, is arguably closer to what the 2015 rules had in mind than the official system that followed it: those rules explicitly assigned restaurant staff a duty to dissuade customers before any inspector got involved. The gap is that a smoker who complies with a fellow diner’s request, out of ordinary social courtesy, is not the same as a restaurant enforcing its own legal obligation, which the 82 percent non-compliance figure suggests almost none of them were actually doing.

Ten years on, the pattern the 2015 law was meant to break, brief, high-profile compliance followed by a near-total lapse once official attention moved elsewhere, looks less like an exception than the default outcome for this particular kind of rule in China.

Researchers who study enforcement point to a structural reason behind numbers like these: responsibility for tobacco control in China is split across several government departments with no single body clearly in charge, which in practice means no one owns the job of actually walking into a restroom to check.
研究执法情况的学者指出了这类数据背后的结构性原因:中国的控烟职责分散在多个政府部门之间,却没有一个部门明确牵头,这在实际操作中意味着,走进厕所去检查这件事,根本没有人真正负责。
Where enforcement does happen, it typically falls to whichever low-authority staff member is on shift, a cleaner, a security guard, a waiter, rather than a uniformed officer with real power behind him. That is a fundamentally different model from the state-backed, high-visibility enforcement China applies elsewhere, and it runs into the same reluctance to cause a scene already seen in wives who hold back rather than confront their husbands directly.
即便真的有人执行,通常也只能靠当班的低权限员工——保洁、保安、服务员——去处理,而不是有实权撑腰的执法人员出面。这与中国在其他领域那种由国家背书、高度可见的执法方式截然不同,也同样撞上了此前在妻子身上看到的那种不愿把事情闹大的顾虑。
A shopping mall restroom is, in that sense, close to the last indoor space in many Chinese cities where none of that machinery reaches: semi-enclosed, rarely fitted with cameras for reasons of basic privacy, and staffed only by cleaners with no enforcement authority whatsoever. It is not that these men disregard rules in general; it is that this particular rule was built to depend on confrontation, in a culture that, as seen already, tends to organise itself around avoiding exactly that.

Face, Generations and an Unmet Target

Surveys find that 97 percent of current Chinese smokers have shared a cigarette and 90 percent have gifted one. Offering a cigarette on first meeting is reported as basic etiquette by over 80 percent of respondents.

The custom draws on older Confucian norms, guanxi (social connection) and mianzi (face), in which a gift signals respect and an accepted gift signals a willingness to build a relationship.

In business and official settings, offering a cigarette still functions as an icebreaker, and declining one can read as a small social rebuff.
